Geriatric Services

Our facility offers a wide range of services for older adults. We have streamlined our admitting and discharge procedures allowing for fast and worry-free admission to our hospitals for patients arriving from nursing homes or assisted-living facilities, including bedside registration.  Our knowledgeable team of healthcare professionals stands ready to provide state-of-the-art clinical care for the elderly patient.  Common geriatric problems treated by our team of professionals include:

Inpatient Programs

Garden Park Medical Center works closely with nursing homes, assisted living centers, home health agencies, and rehabilitation hospitals so that the transition from facility to facility is as smooth as possible.  We have a team of hospitalist physicians who are prepared to accept transfers 24 hours per day, 7 days per week.  Our Emergency Room physicians work closely with physicians admitting from other senior health facilities.  Some other systems used to provide excellent inpatient care include:

Seasons Senior Behavioral Health Center

Seasons is Garden Park Medical Center’s inpatient geriatric-psychiatric program for short-term care. It is a hospital-based program for individuals 55+ who are experiencing emotional or behavioral problems. The 12-bed inpatient unit is on the fourth floor and was specially designed to meet the needs of geriatric-psychiatric patients with a day room, a separate dining area, kitchen and laundry facilities and comfortable patient rooms.   The program provides compassionate, short-term, comprehensive care for older patients in a secure, caring setting.

Individual Treatment Plans

Psychiatrists, physicians, nurses, social workers and other healthcare professionals develop a treatment plan based on the individual needs of the patient. Treatment options help individuals renew social, physical and mental skills. Treatment plans, which are monitored closely and updated when necessary, may include counseling, group therapy and coordinated activities.

Admission Criteria

Admission requirements include persistence of presenting symptoms, a reasonable expectation that the illness will respond to continued treatment and evidence that inpatient hospitalization provides the most effective level of care appropriate for the client.

Nursing Home Programs

To our area nursing homes, we are here to serve you.  You are important to this community and we stand by your side in caring for our seniors.  We provide quality physical therapy, laboratory, and radiological services with fast results here at our facility. 

Outreach services and consultations to nursing homes are offered by qualified professionals who can provide caring treatment to nursing home patients with multiple health issues.  In addition to offering on-site consultation and education, outreach services include on-site blood collection for ordered laboratory tests.
